He begged me to give him pain medication.I knew I couldn’t prescribe painkillers without understanding the situation.The patient told me that he started experiencing foot pain at noon, likely due to eating seafood, which triggered a gout attack.He has hyperuricemia, a condition he discovered last year during a routine check-up at his workplace.Consuming large amounts of seafood at once, especially with hyperuricemia, can indeed induce gouty arthritis.The intense gout pain can truly drive one to the brink of sanity.He pleaded with me to administer pain medication immediately, preferably something that would reduce swelling and pain quickly, regardless of the cost.Before coming to the emergency department, he had visited a community hospital where the doctor prescribed him etoricoxib (an anti-inflammatory analgesic), but it didn’t work well.With the pain worsening at night, he had no choice but to come to the emergency room.Etoricoxib is a good medication, but if it’s ineffective, we’d have to resort to colchicine or glucocorticoids. Especially glucocorticoids, while highly effective, also have significant side effects, so I couldn’t administer them lightly.I asked him to take off his shoes and examined his left foot.With difficulty, he removed his left shoe and sock, and when I saw it, I gasped.Not only was his big toe (first metatarsophalangeal joint) swollen, but in fact, his entire left foot was swollen, with reddened and shiny skin and slightly elevated skin temperature.There was also localized redness and swelling on his ankle. I rolled up his pants to examine his calf, and found it was also slightly reddened.I told them that this was not ordinary gout.
